Dmytri has decades of pharmacy experience, both in hospital and community settings, and holds advanced postgraduate qualifications in clinical pharmacy. Since 2009, he has been credentialed to perform home medicines reviews (HMR). His particular interest lies in the management of chronic diseases, such as diabetes and cardiovascular diseases.

You may be referred for a home medicines review by your doctor. The aim of an HMR is to ensure safe, effective, and appropriate medication use for the patient. After an interview at the patient’s home, a credentialed pharmacist will produce a written HMR report outlining their findings and recommendations. This report will then be discussed between the patient and their doctor, followed by the development of a medication management plan.
